How to Switch to the Choose Life Plate

and How to Apply for Funds

To switch to the Choose Life Plate, take your registration and proof of insurance to the DMV office nearest you. You will pay a one time fee of approximately $50, plus the annual fee of $25, which is tax deductible ($5 goes to the State for processing and $20 goes to the Choose Life program). The funds are distributed to life affirming pregnancy centers, maternity homes and non-profit adoption agencies that assist women with unplanned pregnancies who choose life and adopton for their babies.
